The Scots, who were ahead 16-15 at half-time, pulled away with a Greig Laidlaw penalty, but Italy clinched the win with a penalty try in the last minute.
It was a first win out of three Six Nations games for Italy and a third straight defeat for Vern Cotter's men after narrow losses to France and Wales.
